ABSTRACT

Objective@#We investigated changes in the intestinal flora of children with Mycoplasma pneumoniae pneumonia (MPP).@*Methods@#Between September 2019 and November 2019, stool samples from 14 children with MPP from The Fourth Hospital of Baotou city, Inner Mongolia Autonomous Region, were collected and divided into general treatment (AF) and probiotic (AFY) groups, according to the treatment of "combined Bifidobacterium, Lactobacillus, Enterococcus, and Bacillus cereus tablets live". High-throughput 16S rDNA sequencing was used to identify intestinal flora.@*Results@#Intestinal flora abundance and diversity in children with MPP were decreased. Both Shannon and Simpson indices were lower in the AF group when compared with healthy controls ( P < 0.05). When compared with healthy controls, the proportion of Enterorhabdus was lower in the AF group, while the proportion of Lachnoclostridium was higher ( P < 0.05). The proportion of Bifidobacteria and Akkermansia was lower in the AFY group but Enterococcus, Lachnoclostridium, Roseburia, and Erysipelatoclostridium proportions were higher. The proportion of Escherichia coli- Shigella in the AFY group after treatment was decreased ( P < 0.05).@*Conclusions@#The intestinal flora of children with MPP is disturbed, manifested as decreased abundance and diversity, and decreased Bifidobacteria. Our probiotic mixture partly improved intestinal flora disorders.

ABSTRACT

OBJECTIVE@#To observe the effect of electroacupuncture on motor function and muscle state in patients with primary osteoporosis (POP).@*METHODS@#A total of 60 female patients with POP were randomized into an observation group (30 cases, 1 case dropped off) and a control group (30 cases, 1 case dropped off). On the basis of adjusting lifestyle, caltrate was given orally in the control group, 2 pills a day for 4 weeks. On the basis of the treatment in the control group, electroacupuncture was applied at Zusanli (ST 36), Yanglingquan (GB 34), Ganshu (BL 18), Pishu (BL 20), Shenshu (BL 23), etc. in the observation group, with disperse-dense wave of 2 Hz/10 Hz in frequency, once every other day, 3 times a week for 4 weeks. The time of timed up-and-go test (TUGT) and the value of 10 m maximal walking speed (10 m MWS) before and after treatment were compared in the two groups, and the Young's modulus values of bilateral multifidus muscles in prone position and sitting position before and after treatment were compared by real-time shear wave elastography (SWE) in the two groups.@*RESULTS@#After treatment, the TUGT time was decreased compared before treatment in the observation group (P<0.01), and that in the observation group was shorter than the control group (P<0.01). After treatment, the value of 10 m MWS test was increased compared before treatment in the observation group (P<0.05). After treatment, the Young's modulus values of bilateral multifidus muscles in prone position and sitting position were increased compared before treatment in the observation group (P<0.01); except for the left side in sitting position, the Young's modulus values of multifidus muscles in the observation group were higher than those in the control group (P<0.01, P<0.05).@*CONCLUSION@#On the basis of oral caltrate, electroacupuncture can improve the motor function and muscle state in patients with POP.

ABSTRACT

To analyze the etiology and epidemiological characteristics of gastroenteritis virus in foodborne diseases from three cities in Shandong. From January to December 2017, six sentinel hospitals in Jinan, Yantai and Linyi city of Shandong Province were selected as the research sites. Stool samples of 1 397 diarrhea patients were collected, as well as basic information and clinical symptoms. Duplex quantitative RT-PCR was used to detect Norovirus genogroupⅠ (Nov GⅠ) and genogroupⅡ (Nov GⅡ), Sapovirus (SAV) and Human astrovirus (HAstV), respectively, quantitative RT-PCR was used to detect group A Rotavirus (RVA), and quantitative PCR was used to detect Enteric adenovirus (EAdV). The specific gene of the virus were sequenced and typed. It was compared that the gastroenteritis virus rate in cases with different characteristics and the clinical symptoms difference between the virus positive and negative cases. The median age ((25), (75)) was 23 (1, 42) , mainly male, 57.48% with 803 cased and children under 5 years old, 36.36% with 508 cases. The positive rate of gastroenteritis virus was 33.93% (474 cases), and that of Jinan, Linyi and Yantai City were 32.03% (147/459), 41.54% (189/455) and 28.57% (138/483), respectively (0.001). Nov GⅡ had the highest positive rate, 16.54% (231 cases), which, mainly GⅡ.P16/GⅡ.2 (48.28%, 56/116), peaked in May (24.75%, 50/202) and June (19.59%, 38/194). In patients of gastroenteritis virus positive, 44.51% (211/474) had vomiting symptoms, higher than that of patients of gastroenteritis virus negative (34.13%, 315/923). The difference was statistically significant (0.001). In Shandong Province, the majority of gastroenteritis patients were male and children under 5 years old. Nov GⅡ possessed highest epidemic intensity, and peaked in spring and summer. Viral gastroenteritis had atypical clinical symptoms.

ABSTRACT

Objective To understand the main serotypes, antibiotic resistance profiles and molecular typing characteristics of Listeria monocytogenes(LM) isolated from foods in Shandong Province from 2013 to 2016. Methods The antibiotic sensitivity of LM was tested by broth microdilution method. The serotypes were determined by slide agglutination and PCR, and the molecular typing was carried out by pulsed field gel electrophoresis (PFGE) and multilocus sequence typing(MLST) . Results Most of 191 LM strains were sensitive to the eight antibiotics tested. Tetracycline resistance was most prevalent (15/191, 7.85%). There was no significant difference in the 8 antibiotic resistance monitored for 4 years (P=1.000). The serotype 1/2a, 1/2b and 1/2c accounted for 38.82% (66/170), 18.82% (32/170), and 42.36% (72/170), respectively. The patterns of SDSRZXDZ016, S2014L031 and SDSRZX030, totally accounted for 33.78%, were the dominant types. The main ST types were ST9, ST8 and ST121, which accounted for 81.18% (69/85). The clinical common types, ST3, ST7 and ST87 accounted for 8.23% (7/85), mainwhile new ST type was not found. Conclusion The LM strains isolated in Shandong Province from 2013 to 2016 were sensitive to most antibiotics, but some strains were resistant to tetracycline and erythromycin. The dominant serotypes were 1/2c and 1/2a. Serotype 4b, prone to outbreaks of listeriosis, was not found. The main PFGE types were SDSRZXDZ016, S2014L031 and SDSRZX030, which were continuously found from 2013 to 2016. The main ST types were ST8, ST9 and ST121. The clinical types, ST3, ST7 and ST87 were isolated from food and should be paid seriously attention to.

ABSTRACT

OBJECTIVE@#To compare the therapeutic effect of thunder-fire moxibustion combined with vibration training and simple vibration training on low back pain of primary osteoporosis by 's modulus of ultrasonic wave, and seek an objective evaluating method.@*METHODS@#A total of 60 patients were randomized into an observation group and a control group, 30 cases in each one. The patients in the two groups were treated with vibration training using whole body vibration treatment bed. In the observation group, thunder-fire moxibustion was applied at Yaoyangguan (GV 3), Mingmen (GV 4), Ganshu (BL 18), Shenshu (BL 23) and Dachangshu (BL 25). The treatment was given once every other day, 3 times a week for 4 weeks. The visual analogue score (VAS), real-time shear wave elastography (RTSWE) and medical outcomes study 36-item short-form hearth survey (SF-36) were used to evaluate pain intensity, multifidus muscle tone ('s modulus) and quality of life before treatment, after 4-week treatment and 1 month after treatment.@*RESULTS@#Compared before treatment, the VAS scores, 's modulus of multifidus muscle and 5 dimensions of SF-36 (physical condition, body pain, general health, social function and mental health) after 4-week treatment and 1 month after treatment were significantly improved in the two groups (all <0.05), the physiological role in the observation group after 4-week treatment and 1 month after treatment were improved (both <0.05). In the observation group, the VAS scores, 's modulus of multifidus muscle and 3 dimensions of SF-36 (physiological role, body pain and general health) after 4-week treatment and 1 month after treatment were superior to the control group (all <0.05).@*CONCLUSION@#The therapeutic effect of thunder-fire moxibustion combined with vibration training is superior to simple vibration training in relieving low back pain intensity and multifidus muscle tone, and improving quality of life for primary osteoporosis. RTSWE technique can be an objective examination method to evaluate pain.

ABSTRACT

Objective To study the characteristics and epidemic trend of Shigella in Shandong province through the analysis of serotype, virulence genes, molecular typing and drug sensitivity. Methods The serotype was classified using the method of slide agglutination. Polymerase chain reaction (PCR) was used to amplify the related virulence genes. The molecular typing was carried out by pulsed field gel electrophoresis (PFGE), and the antibiotic sensitivity of the strains was determined by micro-broth dilution method. Results The main serogroups of 44 Shigella strains were Shigella flexneri (54.55%) and Shigella sonnei (43.18%). The carrying rates of ipaH, Set1, Sen and ial were 100%, 43.18%, 56.82% and 50.00%, respectively. By PFGE typing, the strains of Shigella flexneri were divided into 18 patterns with a low similarity. The strains of Shigella sonnei were divided into 14 patterns, and the similarity of 89.47% of the strains was more than 90%. 44 strains of Shigella had different levels of resistance to 14 of the 15 antibiotics. 93.18% of the strains were multidrug resistant. Conclusion The Shigella in Shandong province is dominated by serogroups of Shigella flexneri and Shigella sonnei, with high virulence gene carrying rate, clustering distribution and severe antibiotic resistance. It is necessary to strengthen the monitoring on serotype, traceability and antibiotic resistance of Shigella in Shandong province.

ABSTRACT

OBJECTIVE: To investigate the relationship between pathogenesis of PBB and the T cell subsets disorders and to evaluate the clinical significance of immunomodulatory therapy to the prognosis of PBB and the prevention of recurrent PBB. METHODS: A total of 127 cases of PBB children treated in the Fourth Hospital of Baotou City from May2015 to May 2018 were selected. Blood samples were collected before treatment,and the levels of CD4,CD8 cells and CD4/CD8 in peripheral blood T cell subsets were detected by flow cytometry. All children with PBB(PBB group)were given oral amoxicillin and clavulanate potassium for 2 to 4 weeks. The subjects were divided into Huaiqihuang group(n=66)and non-Huaiqihuang group(n=66). The Huaiqihuang group was given Huaiqihuang granules based on the anti-infective treatment. In addition,healthy children were enrolled as control group(n=39). T cell subsets of the two groups were reexamined 3 months after treatment. By analyzing the cough symptom scores of all children after 2 weeks,4 weeks and 12 weeks of treatment,the rate of cure and improvement and the recurrence rate after 1 year were evaluated.SPSS16.0 software was used for data processing,and the difference was significant if P0.05). In 2 weeks after treatment,the cough symptom scores of the PBB group began to decrease,and the non-Huaiqihuang group decreased more significantly,with statistically significant difference(P0.05). Follow-up showed that the recurrence rate of Huaiqihuang group was lower than that of non-Huaiqihuang group in 1 year after the end of treatment,and the difference between the two groups was statistically significant(P<0.01). CONCLUSION: Children with PBB have T cell subsets disorder,and the treatment of anti-infection combined with Huaiqihuang granules for at least 4 weeks has a positive effect on PBB prognosis and recurrence prevention.

ABSTRACT

Astrocytes are the most abundant cells in the central nervous system, which has been demonstrated to be one of the targets for the treatment of ischemic stroke. Many studies have confirmed that acupuncture can effectively regulate astrocyte activity in ischemic stroke through these approaches: repairing astrocyte morphology structure, regu-lating energy metabolism, inhibiting excitotoxicity, inhibiting inflammation response and promoting nerve regeneration. In this paper, the authors summarized the relationship between astrocyte and ischemic stroke, and discussed the mechanisms of acupuncture therapy underlying improvement of ischemic stroke. Additionally, the authors also put forward some suggestions about future researches on acupuncture in ischemic stroke.

Objective:To conduct empirical analysis on the impact mechanism of local public health expenditure based on fiscal decentralization.Methods:Using panel data of China's 31 provinces from 2010 to 2015,through static panels,FGLS,GMM estimation methods,the influence of different fiscal decentralization indexes on local public health expenditure in China were analyzed.Results:Different fiscal decentralization indexes had different effects on public health expenditure.Local governments with bigger fiscal autonomous income fight might increase public health expenditure effectively.Conclusion:It needed to further differentiate fiscal powers of the central and local governments reasonably,clear responsibility boundary,build the central and local public health expenditure hard constraint mechanism and improve the enthusiasm of local governments to provide unproductive public products.

ABSTRACT

Objective:To explore the association between phase angle (PA) of bioelectrical impedance analysis (BIA) and nutritional risk assessed by Nutritional Risk Screening (NRS-2002) Subjective Global Assessment (SGA),and hospital length of stay (LOS) in patients undergoing thoracic surgery.Methods:60 patients who underwent thoracic surgery were evaluated for nutrition risk and hospital length of stay (LOS) by NRS2002,SGA,albumin and bioelectrical impedance analysis.Results:The PA values were lower in the patients [men(3.85 ± 1.0)°,women(4.9 ± 0.6)°] than the control group [men(6.0 ± 1.0)°,women(5.4 ± 0.9)°,P < 0.01].Patients were more likely to have lower PA values than the control group.NRS 2002:no risk (relative risk (RR) 2.8,95% confidence interval (CI) =1.2 ~ 6.9),moderate risk (RR 3.9,95% CI =1.8 ~ 8.6) and severe risk (RR 4.2,95% CI =2.0 ~ 8.7);similar results were obtained by SGA:eutrophy (RR 2.5,95% CI =0.9 ~ 6.9),Moderate malnutrition (RR 4.4,95% CI =2.1 ~ 9.4),severe malnutrition (RR 3.9,95%CI =1.9 ~ 8.0);Patients with low PA values were more inclined to be hospitalized for more than 21 days compared with the control group (LOS ≥ 21 days,RR =4.4,95% CI =2.2 ~ 2.8).Conclusion:There is a significant association between low PA values and nutritional risk and prolonged LOS.PA is helpful to identify patients who are at nutritional risk at hospital admission in order to provide an objective basis for determining the nutritional interventions of patients and judging the outcomes of the diseases.

BACKGROUND:Knee osteoarthritis is a joint and articular cartilage degenerative disease,and its biological changes mainly include proliferation and apoptosis of chondrocytes.Articular cartilage holds poor regeneration ability,and tissue-engineered cartilage is of great significance for the articular cartilage repair,while cytokines is a major concem for this repair process.OBJECTIVE:To overview the main regulatory factors involved in the articular cartilage repair and chondrocyte apoptosis.METHODS:PubMed and WanFang databases were retrieved for the literature addressing articular cartilage repair and main regulatory factors involved in articular cartilage repair and chondrocyte apoptosis published from 1999 to 2016.Finally 44 eligible articles were included for analysis.RESULTS AND CONCLUSION:Various cytokines in different human tissues are closely related to articular cartilage repair,chondrocyte apoptosis and pathological changes of osteoarthritis,which are involved in chondrocyte damage,degradation of cartilage matrix,synovial degeneration and periostosis.There is an increase in the levels of interleukin 1 β and 6,and tumor necrosis factor α following articular cartilage injury.Thereafter,blocking the expression of these cytokines can protect the articular cartilage from damage.Insulin-like growth factor and transforming growth factor play an important regulatory effect on the chondrocyte proliferation and matrix synthesis.Furthermore,various cytokines regulate the articular cartilage repair and reconstruction via complicated pathways.

ABSTRACT

OBJECTIVE:To provide reference for prescriptions audit before payment in hospital pharmacy. METHODS:The management measures of prescriptions audit and intervention before payment in our hospital were introduced. 1 200 outpatient pre-scriptions within 1 year before and after management were subjected to a statistical analysis. The effects of prescriptions audit be-fore payment were evaluated through analyzing and comparing qualification rate of prescription and unqualified prescriptions. RE-SULTS:Our hospital adopted a series of measure to realize prescriptions audit and intervention before payment by clinical pharma-cists,such as using information audit system,formulating audit standard,educating pharmacists,establishing audit job system, prescriptions review and feedback system,setting up communication system,etc. Of the total audited 189 665 prescriptions, 14 581(7.69%)failed to audit and 606 prescriptions hadn’t been revised after 1 year management,with effective intervention rate of 95.84%. Compared with before management,the prescription qualification rate increased from 88.83% to 98.67% after manage-ment(P<0.01). The quantity of overdose,inconformity with medical insurance,overload and other conditions reduced to 0. CON-CLUSIONS:It is feasible for pharmacists to audit and intervent prescriptions before payment in hospital pharmacy to improve pre-scription qualification and promote rational drug use in the clinic.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of prophylactic administration of all-trans retinoic acid (ATRA) in relieving inflammation in a rat model of collagen-induced arthritis (CIA).</p><p><b>METHODS</b>Female Wistar rats (6 to 8 weeks old) were randomly divided into normal control group, solvent control group, and prophylactic ATRA treatment (0.05, 0.5, and 5 mg/kg) groups. All the rats except for those in normal control group were subjected to subcutaneous injection of type II collagen and incomplete Freund adjuvant in the tails to induce CIA, followed by injection on the following day with saline, corn oil or different doses of ATRA 3 times a week. The arthritis index (AI) scores, histological scores, serum levels of TNF-α, IL-17A, and IL-10, and expressions of proteases related with cartilage damage were evaluated.</p><p><b>RESULTS</b>On the 15th day after the primary immunization, the AI scores increased significantly in all but the normal control groups; the scores increased progressively in all the 3 ATRA groups but remained lower than that in the solvent control group, which was stable over time. The rats in the 3 ATRA groups showed obvious pathologies in the knee and ankle joints, but the semi-quantitative scores of pathology damage showed no significance among them. Compared with those in solvent control group, the serum IL-17A and TNF-α levels decreased, serum IL-10 level increased, and the expressions of ADAMT-4 and MMP-3 proteins decreased significantly in the knees in the 3 ATRA groups.</p><p><b>CONCLUSION</b>ATRA can reduce the production of TNF-α and IL-17A and increase the production of IL-10 to alleviate the inflammation in rats with CIA. ATRA may delay the progression of RA by correcting the imbalance of Th1/Th2 and Th17/Treg.</p>

Objective To achieve outpatient prescriptions audit before payment and dispensing ,optimize the dispensing process ,improve the ratio of qualified prescriptions and promote rational use of drugs .Methods Based on the Hospital Informa‐tion System (HIS) of Peking University Cancer Hospital ,a new computer system was developed for pharmacists to achieve prescriptions audit before payment and dispensing .Results Outpatient prescriptions audit was guaranteed by the new computer system ,the dispensing process was optimized and the role of pharmacists in prescriptions audit was ensured .Conclusion The ratio of unqualified prescriptions was reduced effectively ,and the safety and rationality of prescriptions was improved .Pharma‐cists should improve their professional knowledge to make the prescription audit more professional ,accurate and efficient .

ABSTRACT

To study the hemolytic effect of polyphyllin II (PP II) mediated by anion channel protein and glucose transporter 1 (GLUT1), in order to initially reveal its hemolytic mechanism in vitro. In the experiment, the spectrophotometric method was adopted to detect the hemolysis of PP II in vitro and the effect of anion channel-related solution and blocker, glucose channel-related inhibitor and multi-target drugs dehydroepiandrosterone (DHEA) and diazepam on the hemolysis of PP II. The scanning electron microscope and transmission electron microscope were used to observe the effect of PP II on erythrocyte (RBC) morphology. The results showed that PP II -processed blood cells were severely deformed into spherocytes, acanthocyturia and vesicae. According to the results of the PP II hemolysis experiment in vitro, the anion hypertonic solution LiCl, NaHCO3, Na2SO4 and PBS significantly inhibited the hemolysis induced by PP II (P < 0.05), while blockers NPPB and DIDS remarkably promoted it (P < 0.01). Hyperosmotic sodium chloride, fructose and glucose at specific concentrations notably antagonized the hemolysis induced by PP II (P < 0.05). The glucose channel inhibitor Cytochalasin B and verapamil remarkably antagonized the hemolysis induced by PP II (P < 0.01). The hemolysis induced by PP II could also be antagonized by 1 gmol x L(1) diazepam and 100 μmol x L(-1) DHEA pretreated for 1 min (P < 0.01). In conclusion, the hemolytic mechanism of PP II in vitro may be related to the increase in intracellular osmotic pressure and rupture of erythrocytes by changing the anion channel transport activity, with GLUT1 as the major competitive interaction site.

<p><b>OBJECTIVE</b>To investigate the prevalence rate of childhood asthma in 2010 in urban Baotou, China, as well as the characteristics of attacks and the status of diagnosis and treatment of childhood asthma.</p><p><b>METHODS</b>More than 10 000 children (0-14 years) were selected from 3 secondary schools, 3 primary schools, 6 kindergartens, and 4 community vaccination sites in urban Baotou by cluster random sampling between September 2009 and August 2010. A standardized preliminary questionnaire was used for screening out suspected cases, which were then confirmed or excluded by a clinician; the confirmed cases underwent further questionnaire survey. Double entry and validation was adopted for all data using Epi-Info software, and analysis was performed using SPSS 13.0.</p><p><b>RESULTS</b>A total of 11 323 children were surveyed. Asthma was diagnosed in 127 cases (including 121 children with typical asthma and 6 children with cough variant asthma), with a prevalence rate of 1.12%. The prevalence rate of asthma in male children was significantly higher than that in female children (1.51% vs 0.72%; P<0.01). The prevalence rate of asthma in 2010 was significantly increased compared with that in 1990 (0.55%) and 2000 (0.88%) (P<0.05). Systemic glucocorticoid use decreased significantly from 60.2% in 2000 to 25.9% in 2010 (P<0.01); inhaled corticosteroid use increased significantly from 13.6% in 2000 to 85.8% in 2010 (P<0.01); antibiotic use decreased from 98.1% in 2000 to 66.9% in 2010 (P<0.01). The multivariate logistic regression analysis showed that family history of allergy, allergic rhinitis, chronic cough, and recurrent respiratory tract infection were independent risk factors for childhood asthma.</p><p><b>CONCLUSIONS</b>The prevalence rate of childhood asthma in urban Baotou shows an increasing trend. Inhaled corticosteroids have been widely used.</p>

ABSTRACT

Objective To investigate the epidemiological information of patients in pre-hospital medical care for our large and medium-sized cities and probe the patients' characteristic. Method The data in 2008 were exported from the computer databases of 8 large and medium-sized cities' emergency medical centers in our country.The thorough records of data were conducted to statistical analysis. Results ( 1 ) The scheduling time, running time, rescue time, returning time, total time and service radius in the pre-hospital medical care group were 2.16± 1.10(min), 14.01 ±6.82(min), 12.12±5.96(min), 14.08± 6.85(min), 42.34± 20.21(min)and 8.50±4.18(km), and the above parameter in the non-death group were 2.19 ± 1.13(min), 14.15 ± 7.14(min),11.60±6.72(min), 14.92 ±6.89(min), 41.86± 19.53(minutes) and 8.63±4.31(Km), and the above parameter in the death group were 2.10± 1.08(min), 13.68 ± 7.14(min), 25.25 ± 12.34(min), 13.75±6.48(min), 54.74 ± 25.47(min) and 7.86± 3.91(Km), and the above parameter in the non-sudden cardiac death group were2.09± 1.03(min), 13.58±6.78(min), 25.53± 12.34(min), 13.60± 6.54(min), 53.79±23.77(min) and 7.67 ± 3.86(Km), and the above parameter in the sudden cardiac death group were 2.12 ±1.02(min), 14.10±7.05(min), 24.79± 12.08(min), 13.79±6.61(min), 54. 80 ± 25. 36( min) and 7.90±3.92(Km) respectively. The scheduling time, running time, returning time and service radius in the death group were less than those of the non-death group, but the rescue time and total time of the former were more than those of the latter respectively ( P < 0.05 or P < 0. 001 ). The scheduling time and returning time didn' t have significant difference between the sudden cardiac death group and the non-sudden cardiac death group respectively ( P > 0.05), but the running time, total time and service radius of the sudden cardiac death group were more than those of the non-sudden cardiac death group, and the rescue time of the former was less than that of the latter respectively ( P < 0.05 or P < 0.001 ). (2)The patients' amount in pre-hospital medical care group, the non-death group, the death group, the non-sudden cardiac death group and the sudden cardiac death group were at most in first quarter, and the least time slice of patients' amount were 4:00~ 6:00, 4:00~6:00, 4:00~ 6:00, 22:00~ 24:00, 2:00~4:00 respectively, and the most time slice of patients' amount were 20:00~ 22:00, 20:00~22:00, 8:00~ 10:00, 2:00 ~ 4:00, 8:00 ~ 10:00 respectively. (3)In 241 876 cases of pre-hospital medical care group, the patients' amount of trauma was at most, whose age grades was by far among21 ~50, and the others in sequence were nervous system, circulatory system, other group, digestive system, respiratory system and poisoning group respectively, whose age grades in nervous system, circulatory system and respiratory system was by far above 51, especially above 70. The patients' age grades in other group and digestive system had two climax age groups, which the one was 21 ~ 30, and the other was above 70. The patients' age grades in poisoning group was by far among 21 ~ 50, which the patients' amount of acute alcoholism was at the most. (4) In 12 568 cases of death group, the death amount of circulatory system, other group, respiratory system, nervous system and digestive system ranked at the lst,2nd,4th,5th 8th respectively, whose age grades was by far above 51, especially above 70,and the patients' amount of sudden cardiac death was at the most in the death amount of circulatory system. The death amount of trauma and poisoning group ranked at the 3rd, 6th respectively, whose age grades was by far among 21 ~ 50. (5)The total amount, the death amount and the sudden cardiac death amount of male patients were more than those of female patients. (6)The percentage of the death group to the pre-hospital medical care group was 5.20%, and the percentage of the sudden cardiac death group to the pre-hospital medical care group was 1.29%,and the percentage of the sudden cardiac death group to the death group was 24.87 %, and the percentage of the sudden cardiac death group to the circulatory system group was 67.33 %. Conclusions ( 1 )The trauma and the sudden cardiac death are the overriding reason of disease and the overriding reason of death in our large and medium-sized cities respectively. (2) It is very important to cut the death rate of the middle-old age patients by strengthening prevention and cure of cardiovascular and cerebrovascular diseases, discerning the critical illness early and improving the level of pre-hospital medical care. (3)It is a strong method to decrease the total amount and the death amount of the trauma, especially in traffic accident, by strengthening safety in production, observing traffic regulation and enhancing the legal awareness.

ABSTRACT

Objective To investigate the effects and values of extracorporeal membrane oxygenation (EC-MO) used in patients after cardiac arrest. Method During five years period from June 2002 to June 2007,fifteen cases taken for cardiopulmonary resuscitation were treated by using ECMO in the emergency department and ICU. All the measures for disgnosis and treatment were observed to the guidelines for cardiopulmonary resuscitation and emergency cardiovasculat care set by the American Heart Association in 2005, and ECMO was applied in addition. The study was a self-comparison trial. The biomarkers including heart rate (HR), mean arterial pressure (MAP), central venous pressure( CVP) , arterial partial oxygen pressure (PaO2), arterial partial pressure of carbon dioxide ( PCQ2), oxygen saturation (SaO2), hydrogen power (PH), and concentration of lactic acid were taken and assayed before and 10 min, 1 h,6 h, 12 h, and 24 h after treatment. Differences between the results of measurements were analysed by t -test for matched pairs using SPSS version 10.0 software package. Neurological sequelae was also observed and described. Results Ten minutes after ECMO treatment, MAP rose dramatically (P < 0.01) and got to further higher level one hour later ( P < 0.05). On the other hand, CVP dropped off 10 minutes after ECMO treatment and further lowered one hour later, and PaO2 and SaO2 were improved apparently ( P < 0.01), and pH was increased significantly (P < 0.01), while a derease in concentration of arterial LA of significantly (P < 0.01). Of the 15 cases, 11 patients got well,including completely recovered without sequelae in 5,memory disorders in 2 and hemipleggia in 2 patients. Conclusions ECMO provides effective blood flow to hearts, and therefore, improves oxygenation and oxygen consumption greatly. Moreover, it provides stable oxygenated blood to brain and therefore, it is a good approach to cardiopulmonary cerebral resuscitation.

ABSTRACT

[Objective]To investigate the regular pattern of references quoted in theses of Orthopedic Journal of China from year 2005 to 2006,and to study the characteristics and experiences of reference quoting in Chinese orthopedic academic sphere.[Method]Forty-eight issues of Orthopedic Journal of China from year 2005 to 2006 were downloaded from the website www.chinajournal.net.cn.There were 466 theses about clinical research.All references in these theses were collected as origin of data.Excel software was applied to analyze all references.[Result]References were found in all 466 papers with the reference rate of 100%.The average reference amount in each thesis was 7.15(3 334/466).A total of 3110 references were from journals(93.28%)and 214 references were from monographs(6.42%).The Price Index reached 52.13%.And the number of self-quoted references reached 614 with the rate of self-quoting of 18.42%(614/3 334).References from Chinese and foreign journals with the first 20 highest being-quoting rates were collected,and it reached 56.12% of all references.[Conclusion]The content of references in Orthopedic Journal of China ranges widely.The main quoting origin is from journals.The mainly-quoted language is English,and references from other languages are rarely quoted.Most references showed new and original ideas.
